COST Form
At Dayton, we have a Coordination of Services Team (COST) that meets every two weeks to discuss how to support individual students who may have academic, behavioral, and/or social-emotional concerns. Our COST team looks at forms that are submitted by school staff and parents to determine the next steps to support these individual students.

If you have concerns about an individual student, you can submit a COST form here:

The Dayton COST team members include:
Mrs. Valentino - Principal
Mrs. Barham - School Psychologist
Ms. Davila - Special Education Resource Specialist
Mrs. Grashton - Instructional Coach and Intervention Teacher
